Children / Kids Registration & Liability Form

This form is required for all children participating in activities organized by Williams Memorial UMC Children's Ministry. It serves two main purposes: to provide parental consent for your child’s involvement in our ministry programs, and to grant permission for us to use photos or videos of your child in promotional materials, including social media, websites, and other publications.

  • By signing this form, you acknowledge that you understand the nature of the activities and assume responsibility for your child’s participation. Additionally, this form includes a waiver of liability, which releases Williams Memorial UMC and its volunteers from responsibility for any injuries or accidents that may occur during participation.

Authorization and permission is hereby given by the undersigned to WILLIAMS MEMORIAL UMC to PHOTOGRAPH/VIDEO participant in appropriate manner and USE THE PHOTOGRAPHS/VIDEOS/LIKENESSES OF PARTICIPANT in ANY manner or media that WILLIAMS MEMORIAL UMC chooses, including but not limited to newsletters, websites, or advertising.

LIABILITY AND CONSENT FORM FOR PARTICIPATION IN ALL WILLIAMS MEMORIAL UMC CHILDREN'S MINISTRY ACTIVITIES

In consideration of my (our) child being accepted for participation in WILLIAMS MEMORIAL UMC CHILDREN'S MINISTRY activities at WILLIAMS MEMORIAL UNITED METHODIST CHURCH, I/we, the undersigned, acknowledge the inherent risks involved and hereby release, discharge, and hold harmless WILLIAMS MEMORIAL UNITED METHODIST CHURCH, its directors, staff, and volunteers from any and all liability, claims, or demands for personal injury, illness, or loss arising from participation in any activity, trip, or event.

I/we understand and accept the risks of injury, illness, or death associated with participation and agree to assume full responsibility for all such risks. I/we also authorize the church to provide necessary transportation, food, and lodging for my (our) child.

In the event of an emergency, I/we authorize church staff to seek medical treatment for my (our) child. If I/we cannot be reached, I/we give permission for a physician selected by the church to administer treatment, including hospitalization, injections, anesthesia, or surgery as needed.

I/we agree to assume responsibility for any transportation costs if my (our) child needs to return home due to medical, disciplinary, or other reasons.

I/we agree that if any person or persons are listed on this form that they are authorized by me/us to pick our child up from any and all events put on by WILLIAMS MEMORIAL.

FURTHERMORE, BY SIGNING BELOW I UNDERSTAND THAT I CERTIFY ALL THE INFORMATION ABOVE TO BE CORRECT, AND REVIEWED BY ME/US.

SHOULD ANY INFORMATION ON THIS FORM CHANGE, I UNDERSTAND THAT IT IS MY RESPONSIBILITY TO IMMEDIATELY UPDATE WILLIAMS MEMORIAL UMC WITH THE MOST UP TO DATE INFORMATION REGARDING THE MINOR(S) ON THIS FORM AND THAT UNTIL I DO SO, WILLIAMS MEMORIAL UMC WILL USE THE INFO LISTED HERE AND WILL NOT BE AT FAULT IN ANY WAY SHOULD THIS NOT BE THE MOST UP TO DATE INFORMATION AT THAT TIME.

Assist in Sunday school classes or small groups

Support with set-up, clean-up, or administrative tasks
No experience is required — just a loving heart and a willingness to serve.

Friendly Note: All lessons are pre-prepared for you. You will not be asked to prepare lessons/crafts for bible study!
